Changes are made to our formulary each new plan year. Below is an abridged Formulary Alternatives list for medications removed from the formulary in 2010. Please also look at the 2010 Abridged Formulary included in your Annual Notice of Change (ANOC) packet to determine if you are taking any medications that have been removed.

Please print this page and take it to your doctor with your new formulary to obtain a prescription for a formulary approved alternative. By moving to a formulary drug, you will avoid paying full cost for a non-formulary medication.
